• Conduct telephonic outreach for medication adherence to identify barriers.
• Conduct efforts to convert extended day prescriptions.
• Task physicians and staff for care gaps.
• Request records to fulfill care gaps.
• Contribute to collaboration for best practice with employed and embedded staff.
• Mailings as required for campaigns or wellness visits.

Job Requirements:

• High School Diploma or GED
• Basic computer knowledge
• Basic understanding of medical terminology.
• Compliance of policies, procedures and regulations to ensure patient safety

Preferred Job Requirements:

• Knowledge of medical terminology, experience with billing and/or ICD-10 coding, or Medical Assistant or Pharmacy Technician experience preferred.
